When ordering by E-Mail or Fax, please be sure to include:

• Name, phone number, billing address, shipping address, and include the recipient’s name, if applicable.
• Catalog number and requirements (download sample order PDF ).
• Proper abbreviation for Services (e.g. Defibrinated Plasma – DFP).
• Quantity, preferred packaging and payment information (whether you are paying by credit card, wire transfer, or would like to be billed, our terms are Net 30 days).
• Date needed, special requests/instructions and purchase order number if available.
